We can assist with care for your children from any age,from birth through adolescence whether that means assisting with bathing, feeding, grooming, personal care and nurturing your child’s inquisitive mind.
Our Home Health Aides and Personal Care Assistants can help you or your loved one keep up with the activities of daily living whether it be bathing, grooming, dressing, toileting, meal preparation, transferring and ambulating, safety and fall precautions, monitoring vital signs and medication reminders. We can help with the errand shopping, light housekeeping, escorts to medical appointments and just offer warm companionship.
Unicare’s Alzheimer and dementia care programs focus on providing caring and sensitive companionship and assistance such as meals, errands and transportation, walking and mobility. We make it a point to keep our patients mentally stimulated and assist their memory preservation through activities such as reading, conversation, music listening, painting and games.
We can provide you with home health care coverage for a few hours, during the work day, morning, evenings, splits shift care or continued care for 24 hours.
Nurse case managers can fill a great number of roles - they can be your social worker or patient welfare advocate and often serve as a liaison.

Self-directing persons or someone who has and is willing to make decisions about the services being provided
CDPAP, or Consumer Directed Personal Assistance Program, is a unique Medicaid program that provides the elderly or disabled the ability to manage their own care and make decisions based on their individual needs.
This program empowers the individual to reclaim independence and comfort in their life, while choosing someone they know and trust. The patient maintains total freedom of choice in setting their care schedule and directing how and when their plan of care is managed. CDPAP is a personal way to get the help you need at home without having to deal with the concerns that come with hiring a HHA.
How Does CDPAP Differ From Traditional Home Care?
In most home health care situations, there is no level of personal connection previously established between the patient and home care assistant. With the Medicaid CDPAP program, the individual in need can receive care from a friend or family member with whom they trust and are comfortable with, in order to provide the best possible home care personalized for the patient. This allows the patient to maintain a sense of independence, while still receiving the assistance and level of care needed.
We work with Medicaid, Medicare and most insurances as well. We have private pay options as well.
We service the New York Metro Area: Brooklyn, Bronx, Queens, Manhattan, Staten Island and Nassau County. For CDPAP services – we can act as the Fiscal Intermediary in all of New York State.
Unicare carefully screens each applicant via an extensive background check and testing of skills, knowledge and relevant abilities. We use a verified and reputable telephone attendance system that allows us to monitor attendance and if services are being rendered appropriately. Each caregiver responds to a team leader whose responsibility is to monitor aide professionalism and provide support to both the Unicare caregiver and the patient/patient’s family.
